I've used a similar template myself to fill out for my team members and let them fill it out themselves, then we would meet, discuss the deltas in scores and see how some of these scores/oppoprtunities can be formed into a hard personal OKR for this individual and their growth plan, and compare evert quarter to previous scores to see improvements.
